福岡の地は、古には大陸からの文化の窓口となり、また現在も北アジアとの交流の窓口でもあります。最近の世界情勢の中にあって、この地で本学会を開催させていただくことには感慨深いものがございます。
近年、気管支喘息はその治療の進歩とガイドラインを中心とした標準的治療の普及によって、喘息死は減少し、臨床経過も改善され、良好にコントロールされるようになってきました。しかし、未だ、ニアミスに近い喘息発作で急患室を受診する患者さんは存在します。
小児科医として考えると、先ず発症させないことが大切です。素因を持っていることは、現時点では変更ができませんが、気道過敏性が獲得されないようにすることは可能ではないかと考えます。発症してしまった後は重症化させないこと。重症化してしまった場合には、これを何とか軽症化させること。そして、思春期を経て成人喘息に移行させないこととなります。成人に移行してしまった後も、軽症化させることと、QOLをできるだけ良好に保つこと、できれば寛解に持ち込むことが目的になります。これらの過程に影響する合併症の問題も考えたいと思いました。また、慢性疾患としての喘息のコントロールには心理的問題はアドヒアランス/コンプライアンス、またQOLとも関連して重要です。心の問題も取り上げさせて頂きたいと考えています。重症化してしまった患者さんの治療も最近急速に進歩しました。生物学的製剤は開発が進み今更ながらアレルギーの関与の重要さが見直されています。また、免疫療法もこれから再び普及して行くと考えられます。これらの治療法の何れを、どのような基準で選択していけば良いのかは明らかではありません。これらの治療法と共に、従来のステロイドの吸入方法も見直しと今後の展開も興味あるところです。新しい検査も今後どのように位置づけられていくのか、この他にも興味の尽きる処を知りません。2日間ではこれらのほんの一部しか語り合うことができませんが、今年も一般演題を募集させて頂きます。テーマは見直しとこれからの治療戦略としましたが、少しでも楽しく語り合うことができ、今後の気管支喘息の研究・治療に結びつくことができればと考えております。

Welcome to the 26th Congress of INTERASMA Japan/North Asia
It is a great honor for us to organize this congress in Fukuoka.
The INTERASM specializes in asthma and the past annual congresses have always been highly productive and successful.
Fukuoka was a window on continental culture in the old days, and even now, it is a site of cultural exchange with North Asia. In the current world situation, it is deeply meaningful to hold this Congress in Fukuoka.
The death rate from asthma has decreased, the clinical outcome has improved, and asthma control has become more favorable with progress in treatment and wider adoption of standard therapy based on guidelines. However, even now, we experience severe acute asthma attack, difficult and complex cases with poor prognosis.
As a chronic diseases, psychological problems influence the control of asthma in association with adherence/compliance and patient’s QOL. Emotional problems are important as well.
Treatment of severe asthma has recently made rapid advances. Development of biological products has progressed, and the importance of allergy is now being re-evaluated. Immunotherapy will probably become widespread again in the future. However, it is not clear which of these therapies should be selected or what selection criteria should be used. We are interested in such therapies and we will also review traditional inhaled steroid therapy and assess future developments. We also have a continuing interest in various other subjects including the position of new tests/examinations.
This time, we can get the opportunity to listen to a series of valuable lectures by outstanding figures from abroad and Japan covering many topics.
We would like to call for abstracts on a wide range of subjects relating to asthma . We decided to have the congress on the theme, “Review and Revitalization of Future Treatment Strategies”, with the aim of providing enjoyable and insightful discussion about future trends in the investigation and treatment of bronchial asthma.
We will do our best to ensure that everybody is satisfied. Please visit us in Fukuoka. The climate is comfortable here in September. Furthermore, the next day (September 19) is a holiday in Japan. I hope you can enjoy bathing in hot springs (onsen) and eating seafood in this area.
